Travel Document for Minors
To make an appointment or to receive the travel document by mail, please submit e-application. In order to receive the travel document by mail, please indicate "postal service" when submitting e-application.
Travel Document for Minors:
Travel documents issued for minors and persons with disabilities will be considered based on the application submitted by the legal representative. In order for a decision to be reached, a written letter of consent is necessary from a parent or legal guardian.
An application form (only necessary when requesting the travel document by mail) must be filled out in Georgian language. It is important to note that all questions on the application must be fully completed and the application must be signed by the legal representative of the minor.
A letter of consent from a parent or legal guardian must be presented at the Consulate General. If a parent or legal guardian is able to visit the Consulate General in person, he or she will be able to complete a letter of consent at no cost.
In the case that a parent or legal guardian is not able to visit the Consulate General, the letter of consent may be certified by a notary office in the host country and then certified by an apostille (List of issuing authorities of apostilles). The letter of consent should be sent along with the following documents:
- 1 Passport size photo (3X4 cm);
- Legal representative’s and minor applicant’s passport copies (if applicable).
There is no service fee for issuing a travel document for a minor.
The documents mentioned above can be submitted in person at the Consulate General or sent by mail. Documents sent by mail, must be accompanied by a prepaid envelope with a return address.
