To make an appointment or to receive the travel document by mail, please submit e-application. In order to receive the travel document by mail, please indicate "postal service" when submitting e-application.
Travel Document for Adults
An application form (only necessary when requesting the travel document by mail) must be filled out in Georgian language. It is important to note that all questions on the application must be fully completed and signed by the applicant.
The following documents must be presented at the Consulate General:
- Copy of the applicant’s passport (if applicable);
- A money order in the amount of $35 U.S. dollars written out to the Consulate General of Georgia in NY;
- 1 passport size photo (3X4 cm).
The documents mentioned above can be submitted in person at the Consulate General or sent by mail. Documents sent by mail, must be accompanied by a prepaid envelope with a return address.
